We appreciate Dr. Singh's comments on our article (1). Although aminophylline, postural drainage, and chest physiotherapy may have a role in some patients with acute exacerbation, their benefits remain largely unproven, and we would therefore not recommend them for all patients with an acute exacerbation. We agree with the components of the treatment package for hospitalized patients mentioned by Dr. Singh. However, we believe this treatment should be individualized rather than provided in a standard package, in both outpatients and hospitalized patients.
