Foreword: The Natural History, Pathogenesis, Evaluation, and Treatment of Emphysema: Report of the National Emphysema Treatment Trial Research Group

Message from the Chair of the NETT Steering Committee

The NETT Steering Committee, composed of the principal and co-principal investigator from each of the 17 clinical centers (primarily thoracic surgeons and pulmonologists) and the coordinating center, the NHLBI project officer, and the study chair, worked collaboratively to write and execute the NETT protocol, to analyze and interpret the results, and to report the results. The Steering Committee could not have conducted the NETT without the strong support and full cooperation of a large team of staff at each of the participating sites. The greater NETT Research Group consisted of about 350 physicians, nurses, pulmonary function and rehabilitation staff, data entry staff, and many others, all contributing to the success of NETT. The complete NETT credit roster is listed at the back of this issue of PATS (pp. 572–574). Over 500 rehabilitation satellite centers worked to provide the NETT rehabilitation to patients near their homes.

The cooperative efforts of this diverse group of investigators with a broad range of expertise and research interests have yielded a vast quantity of valuable information relevant to treatment for emphysema. Through its main protocol, the NETT provided data on the survival and clinical outcomes out to 3 to 5 years after LVRS compared with medical treatment, and characterized the patients likely to benefit from LVRS and those likely to be harmed. Through its substudies and ancillary studies, the NETT also provided information on patient care and management, surgical approaches, ancillary clinical outcomes, genetics of emphysema, pathology of emphysema, and radiographic characterization of the patient. I congratulate the group on their remarkable accomplishments.
